Match the reference and the date
The reference number and date of issue are printed at the head of every report this practice issues. Both must match the record above exactly. A document on this letterhead that carries no reference number was not issued through this register, and should be treated as unverified.
Confirm the subject’s particulars by asking, not by reading

Check that the file has not been altered

What this page establishes — and what it does not

Their soundness is a matter for whatever further assessment a recipient authority considers appropriate, and nothing on this page speaks to it.
Confidentiality
The clinical content of the report — the presenting history, the diagnostic impressions, the severity assessment and the recommendation — is not reproduced on this page, and will not be disclosed by telephone or by email to any party other than the client or a person holding the client’s written authority.
This omission is deliberate, and it should not be read as evasiveness. A verification link is by its nature accessible to anyone into whose hands it passes, including onward recipients the client never contemplated. Publishing identifiable clinical information at such a link would breach professional confidentiality regardless of how legitimate the interest of the first reader may be.
Confirming that a document is authentic and disclosing what it says are two separate acts. Only the first is undertaken here. A recipient who requires the second should apply to the client, who may authorise release in writing.
Status definitions
The status field above will read as one of the following.
- Valid
- Issued by this practice, not withdrawn, and current as at the date of reading. May be relied upon.
- Superseded
- Replaced by a later report on the same matter, ordinarily following clinical review. The reference of the successor document is available on request. The superseded document remains a true record of the opinion held at its date of issue.
- Withdrawn
- No longer to be relied upon, whether because it was issued in error, because material facts have since changed, or at the client’s instruction. Should not be actioned.
- Expired
- The scheduled review date has passed without re-issue. The document remains genuine but its clinical conclusions should not be assumed to describe the client’s present condition. Request a current report.
